Entry Forms

Multiple entries per catagory are allowed for an additional $10 per entry. Complete and click "Send" on both the Entry Form & the Shirt Order. The 1st five shirts are included in the base $50 entry fee. Please get these Forms sent in by September 10th. The $50 secures your commitment. The Shirt Form helps to make sure the correct sizes are available.
